And then you have all the crap that's in orbit right now. Like paint chips, for example. This article is from 1997 and it looks like there were individuals who were genuinely concerned with the orbiter's ability to avoid small-scale collisions.
Flash forward to 2003 and Columbia burns up in the atmosphere after being struck with a piece of foam during liftoff.
Scary, huh?
